Below you will find a list of requirements that must be submitted prior to commencement of any work.

NOTE: We are required by the Internal Revenue Service to have your Taxpayer Identification Number on file each year. If we do not have this information, we may be required to withhold 31% of payments made to you as withholding for federal income taxes.

  • Copy of Contractors license (if applicable),

  • Copy of Janitorial Services Registration (if applicable)

Insurance Documents

All certificates of insurances must use Fasteam, Co. corporate address: 425 W. Rome Blvd North Las Vegas, NV 89084

General Liability - certificate

  • Must name your company and Fasteam, Co DBA FAST, Co. (Do not use C/O)

  •  Must indicate that Fasteam, Co DBA FAST, Co. are listed additional insured’s. (Do not use C/O)

    General Liability – additionally insured endorsement

  • Provide additional insured endorsement supporting the certificate. (If the endorsement is scheduled, each specific party must be named and is not contingent on contractual obligation)

Workers Compensation – certificate

  • Must name the company and Fasteam, Co. . DBA FAST, Co.as certificate holders (Do not use C/O)
